"I recently learned that I have a chronic disease..."

I was told by a doctor that I have a chronic condition, diabetes or chronic heart failure (CHF). My partner also has chronic obstructive pulmonary disease (COPD). We need help figuring out what to do next and how we can get emotional support.

"I do not feel safe in my relationship or at home anymore..."

My partner has changed. They used to be so gentle and kind but as we have both become older, they are now different, mean and violent. I don't know what to do.

"I am pregnant but I don't have a family doctor..."

I am three months pregnant with my first child. I recently got an Ontario Health Insurance Plan (OHIP) card, but I do not have a family doctor. I do not know to access the care I need for me and my baby.

The health care system is very different here in North York and Ontario. Where should a newcomer start looking for help and information? What do I do if I am looking for refugee and asylum claimant support?

"My father is getting older and depending on me more for everything. I feel so much pressure to make sure he is okay but I am overwhelmed..."

Until recently, my father was independent and able to look after himself. After a hospital stay, he is asking for more of my help. I am worried that I am not doing a good job taking care of him and I don't know where to get help.

"My family and I are new to North York and feel like there is so much we need to learn about the systems here..."

We have two children, ages 2 and 6, and I am pregnant. We just moved to North York from another country. We do not know anyone here and want our children to go to school. We do not know what the children need before they can go to school.
